Mission
It is the mission of Community Health Centers of South Central Texas, Inc. to operate a non-for-profit health related entity for the promotion of the health of the community.
Vision
CHCSCT has a vision to offer preventive and primary care services and treatment of the sick, injured, afflicted, disabled, without regard for any person's race, color, creed, national origin, sex, age, religion, economic status, or disability.
CHCSCT has a vision to expand its facilities and services on a timely and appropriate basis to meet the medical needs of the community as support and facilities are available.

Values
CHCSCT values the need to provide quality and comprehensive, preventive and primary care throughout the entire lifecycle of the individuals we serve, who in general, may not be able to receive care elsewhere, especially patients with Medicaid, Medicare, and those who are medically un/underinsured.
Our History
CHCSCT originated in its first form in 1966, as a small health center known as the “Migrant Clinic” in Gonzales, Texas. Born from a growing need for health care for migrant workers and other low-income workers who needed services, the clinic quickly grew to become a chartered health center in 1974.
Starting in Gonzales County, CHCSCT’s service area has grown to include Gonzales, Caldwell, Bastrop, Guadalupe, and Victoria Counties. Serving over 20,000 patients each year, CHCSCT has become an emblem of healthcare excellence in a region designated as a Medically Underserved Area.
From being chartered in 1974 to its transformation into a Federally Qualified Health Center, the organization has evolved and grown, always staying true to its mission to promote community health through accessible, comprehensive, and quality healthcare services.
Accreditations
At Community Health Centers of South Central Texas, we hold ourselves to the highest standards of care, quality, safety, and trust with our patients and partners. Our organization is classified as a non-profit health services provider and holds several accreditations that speak to our commitment to excellence in all we do. Our certifications include:
- Joint Commission – granted to health organizations that meet the highest standards of safety and quality.
- Patient Centered Medical Home – granted to healthcare providers which follow a standard of care that continuously makes improvements to make sure that patients are at the forefront.
- Federal Torts and Claims Act (FTCA) Deemed Organization – As a FTCA deemed facility, our organization receives funding from the U.S. Department of Health and Human Services and has federal Public Health Service deemed status with respect to certain health or health-related claims. This includes medical malpractice claims, for itself and its covered individuals.
- Advocacy Center of Excellence (ACE) Silver Status -This program recognizes and rewards CHCSCT for excellence in formalizing our advocacy program and making it an institutional part of our health center operations.
